Age limit and assisted fertilization (IVF)

We get a lot of questions about Who can seek assisted reproduction? and especially to what extent we have absolute age limits for assisted reproduction (IVF). This article says something about how we handle this topic here at Medicus.

The most current issue is related to the upper age limit for assisted fertilization (IVF), but the lower age limit has also been a topic several times.

In the preparatory work for the Biotechnology Act, the Ministry of Health has provided guiding age limits for assisted reproduction. The woman's age should be over 25 years and under 38 years, and there must be a reasonable age difference between the man and the woman. The background to the recommendation is primarily to safeguard the child that may be born as a result of the treatment, but also the couple who will undergo the treatment, which for some can be a mental and physical burden. When the doctor has made a medical assessment of the couple and a psychosocial assessment of the couple's caregiving capacity, a doctor can offer treatment even if the woman's age falls outside the ministry's recommended age limits.

This means that couples who are married or cohabiting in a marriage-like relationship and who cannot have children on their own can be offered assisted reproduction even when the woman's age is less than 25 years or more than 38 years.

We look at whether the couple's relationship is stable and whether the conditions are otherwise conducive to the child being cared for in a safe and good manner. This means that, when the conditions are conducive, we can also treat couples where the woman's age is, for example, 20 years, if the legal requirements for assisted reproduction are otherwise met. Similarly, we can offer treatment to couples where the woman's age is 40 years or older if the chance of success is acceptable and the couple is otherwise aware of the chances they have of success.

The new Biotechnology Act in 2020 also set an absolute upper age limit of 46 years for women, which means that assisted fertilization of any kind cannot be performed on women who have reached the age of 46.

For men, we practice an upper age limit of 65 years, but here too we make an individual assessment.

In summary:

There is no absolute lower age limit for assisted reproduction.
The upper age limit for women is 46 years.
The upper age limit for men is 65 years, but here too individual assessments are made. The age difference between the man and the woman may also be given some weight.

Hi, my husband and I have finally gotten pregnant through IVF treatment, after trying for 7 years! I am 39 years old, my husband is 45 years old, my question is, how long after giving birth can we start IVF treatment again? As we want two children.

  So great that you've gotten pregnant after so many years of trying. Enjoy your pregnancy and the baby when it arrives!
  You must have regained your cycle after pregnancy and breastfeeding before you can start trying again. How long it takes depends on how long you breastfeed exclusively or breastfeed so often that you don't have a cycle.
  The same will apply if you have frozen fertilized eggs that need to be thawed and put into a freeze/thaw cycle.

We are both over 30, have been together for the last 12 years. Man and woman. We want children now, but feel that it will take too long. Can we get help with IVF either publicly or privately without the reason being fertility problems, but only that we want more children at once due to age, for example?

  Your age isn't dramatically high when it comes to the possibility of having children, is it?
  If you are both healthy and without known problems, you should normally try on your own for 12 months. If you have not succeeded by then, an assessment of both and counselling regarding treatment is recommended. You can then be considered for IVF if you are married or have been in a stable relationship for at least the same period.

I am a 42 year old mother of 2. My husband and I are trying to conceive our 3rd child. Sistemann was first conceived 6 years ago, but we have just started trying. My husband is also 42 years old. Due to his age, I do not want to try indefinitely. I have not measured my egg production. I have regular menstruation of about 27 days. What do you recommend? I would like to get started quickly

  IVF treatment is usually only considered if you have not been successful in getting pregnant on your own after up to 1 year of trying. Various factors can shorten this time. If you are unsure about this, I would advise you to contact a fertility clinic for further recommendations. In Norway, it is generally not possible to check the genetics of the embryo before transfer into the uterus.

Woman, 41 years old this summer. Tests two years ago showed AMH at 3 and FSH at 10. Overweight, but no other known fertility challenges. As a single woman, I bought a contract for three IVFs in Denmark. I had two attempts last year and got pregnant on one. In the fall, I was diagnosed with MA at week 8. I have since gotten a boyfriend, he has no known fertility problems. We have talked about actively trying, but work conditions make it very difficult to achieve ovulation. Would we benefit from using the last IVF attempt together (long protocol), or could it possibly cause hormonal challenges that could get in the way of trying during the months when work is available? The eggs definitely don't get any younger.

  There are several factors that mean you don't have as much time, with age and low AMH being the most important. IVF would probably be the smartest choice in your situation, in order not to potentially lose a lot of time - which in your case could be crucial. You write that you have now got a girlfriend, and I interpret that as meaning that this relationship is quite new. When it comes to possible treatment in Norway, you must either be married, or live in a "marital-like relationship" before you can be offered treatment. "Marital-like relationship" usually means cohabiting with a common address for at least 1 year. If you are planning IVF treatment at a clinic in Denmark, I actually don't know how they feel about this, so you will have to discuss it with them. Good luck!

Sterilized me because I react poorly to contraception.
I now want another child with my new husband of 54 years, and he doesn't have children now, but lost his son many years ago.

Hi, I am 38 years old and want assisted reproduction. I have lived with the man I want to have children with for two years, but for the last three years we have not had the same registered residential address because he works abroad. He is a foreign citizen (European), and in these years we have in reality been partly cohabiting, partly commuting (mostly him). Do we meet the requirements for a marriage-like relationship? Or should we get married to be on the safe side (we are ready for anything!). Thanks in advance!

  If he has "lived abroad" for the last 3 years, it cannot formally be equated with a marriage-like cohabitation. If you are married, you can start treatment at any time.

Hi! I am a 24 year old girl and I have a dream of getting pregnant. I am lesbian and single.
I see on the website that you should be over 25 years old to be "approved" for treatment, does that mean you are strict on this particular rule or is there an option for us just under 25?
A year ago, my then partner and I were well underway with family planning, but that relationship fell apart when she wasn't ready for this.
For years, my biggest dream has been to become a mother, and now I feel that desire is stronger and I long for a child that I can have the honor of becoming a mother to.

I would really like to be considered!

  Hi Frida. Age 25 years is only a guideline, the doctor may decide to treat younger women than that, which we also do.
  However, we cannot treat single women in Norway yet. In addition, the law requires that those who are to be treated are married or live in a marriage-like cohabitation. So age is probably the least of your problems when it comes to treatment in Norway. As a single person, you can seek treatment in Denmark, for example.

I've been thinking for a few years about getting help with pregnancy, if I'm unfortunate enough to end up alone. I read that you can't get help getting pregnant as a single person in Norway, and that many other countries are also strict about this.
Do you know how Denmark deals with this?

Child(ren) are something I will have one day (as long as my body allows), and there are always ways to fix it, but I prefer to do it planned and "under supervision".

  Hi Mathea. Denmark offers treatment for singles as it is today. It may well become legal in Norway in a few years, but that is a political question. It is probably very neat and easy to do it "under supervision" rather than more informal arrangements. We can also assist with investigations or any ultrasounds you may need during the treatment, but we are not allowed to refer or get involved in the treatment through direct contact with the clinic you may choose in Denmark. Hello again Terry,
Thank you for your previous response. I am now 43 years old and will be 44 in September. Unfortunately, I never got pregnant naturally, now 1 year after I had a voluntary abortion at 42.
I am now in Denmark and have had 12 eggs removed after an IVF attempt. A total of 8 eggs have been fertilized! Is this a lot/good for my age? I am so terribly nervous, because I know that after egg implantation it is more uncertain. Unfortunately, most eggs fall out after you turn 40.
I measured 14 eggs on each side just before I started my hormone therapy (Gonal-F and Synarela). My gynecologist claims that the number of eggs is about 3 times more than what is normal for women my age. Hormone values are these: AMH: 14. TSH: 1,2. FSH: 10. Prolactin: 268. Estradiol: 0,25. Testosterone: 1,7.
What do you think about these hormone values considering age?

  You have plenty of eggs for your "age" and fertilization also works very well. The genetic content of the eggs will be largely damaged, which is why so few eggs continue normal development, attach and become children. The hormone values are normal, but the estradiol value is too high to be able to say anything for sure about your basal FSH, but I think it will be elevated when the estradiol value is 0,10 nmol/l or lower. In that case, it means that the ovarian quality is reduced even if the capacity is good.

  Unfortunately, you have to go abroad to get help to have children. Unfortunately, Norwegian politicians do not think that we should offer such treatment in Norway, even though single people can in principle adopt children. However, there are few countries that accept applications from single people. In practice, single applicants are often not given priority, as the authorities prefer to find families that give the child two parents.

Hey,
I will be 43 in two months. When I was 42 and 3 months old, I got pregnant after two weeks of trying with a 31-year-old man. I had an abortion 3 months later. something I have struggled with for months and now think that it is too late. Will there be a greater chance for me to succeed with IVF treatment since I have been pregnant naturally for almost a year? I have checked my ovaries. There are 10 eggs on each side, all hormone values etc are good, says the gynecologist. He says the egg production is comparable to a woman of 30. Will the quality of the eggs be better if you produce many, or does it not matter?
Last but not least: how big is the chance of succeeding again after turning 40, if you have already managed it after turning 40?
I'm desperate. The man I have now is 45. It also makes me uneasy.

  You have a high egg reserve considering your age. The number of genetically abnormal eggs will still be significantly higher than for women under 40 and there will be longer intervals between each time a genetically normal egg is fertilized. You don't need to worry about your husband's age yet, provided that his sperm quality has been tested and found normal. At your age, if the conditions are otherwise suitable, I would recommend IVF with Eeva test to find the most normal embryo to transfer so that you have the highest possible chance of getting pregnant quickly.

  I can add a little here too: Statistically you have a 3% chance of getting pregnant during the 3rd year. The chance in the first year is 87%, the chance in year 2 is 6% and the 3rd year is 3%.   Prices will vary slightly depending on the type of treatment you may need. Price list can be found here. There is a fertility clinic at the hospital in Telemark. We can also help couples no matter where they live. That said, there are no clinics in Norway that help you have a child of a specific gender.

  As a sterilized mother of 3 children, you should choose IVF. You must either find a partner you want to live with in marriage or a marriage-like cohabitation. Then you will be able to receive treatment in Norway. Or you must choose treatment as a single woman, with donor sperm, then you must seek treatment outside of Norway.

My boyfriend and I live in a safe and stable relationship, are cohabiting and engaged with plans to get married. I am 23 years old, he will be 26 in the fall. We have talked about having children for a long time, also tried physically to make it happen, but it has not been successful yet. I have a somewhat strained relationship with sex, so we have now decided to go for test tubes and assisted fertilization. Then another question arises; I was hit by a car in 2008, this ended with a coma and head injuries. The head injuries have again affected the right side. Everything is as it should be inside, I have also been to a gynecologist as I have a metal plate in my pelvis that I wondered if was in the way due to a possible pregnancy, but this was not in the way. I am completely up and running mentally, it has mostly affected my body, something in my voice and vocals. Do you think this will put an end to me? We've been trying for quite some time now, and I don't really want to show up at their offices full of hope, and I'll get rejected. Hoping for a positive response! :)

    If you have been taking Cerazette since middle school, I can understand why you have not been able to get pregnant! Jokes aside, for us it is important that pregnancy and childbirth do not pose a health risk beyond normal for the woman, that the couple lives in a stable marriage-like relationship and that they can take care of the children, now and in the future, and that the treatment can be carried out as normal.   Yes, you can get help. Both the duration of your cohabitation and the time you have been trying meet the requirements to be able to receive an offer of assisted reproduction from us. We need additional information before we can give you such an offer as there are other factors that are important. Submit "patient information", then we will come back with our assessment. Hi! We have been dating for 1 1/2 years and haven't lived together for more than a few months. We have had Insemination in Denmark without success. Have now been trying for a year.   Of course you can get help in Norway, public or private. You must accept treatment with a non-anonymous donor and must be married or cohabiting in a "marital-like relationship", which is often defined as cohabiting at the same address for 2 years.

  I definitely think your husband should have his sperm quality checked. It's possible that it has changed since he became a father last time. Maybe it's normal, but it will take a long time before it's so bad that we can't help you have children.

  Women in your situation will be able to have children, but will need to get eggs from another woman. The egg is fertilized with your partner's sperm and returned to your uterus so that you are the one who becomes pregnant, carries and gives birth to the child. It is not possible to get any closer to genetically your own child! Such treatment cannot currently be offered at clinics in Norway, but there are many good clinics that can help you.   We do not treat women over the age of 45. The reason for this is that we have never been able to help anyone in that age group have children.
  
  If you want to find out if, in addition to age, there are other conditions that make it difficult for you to get pregnant, you should see a gynecologist with expertise in infertility evaluation to have hormone tests done including AMH, ultrasound examination of the uterus, ovaries and fallopian tubes and a semen analysis of your partner, as described on our website under "Investigation of childlessness".

Hi! I am 19 years old (in May), and my fiancé is 20 (in March). We have been trying for 9 months without success. I went to Volvat for an evaluation due to long cycles, up to 90 days. It was confirmed that I am not ovulating using blood tests and ultrasound. But no more help and no. I know that pergotime has an effect on many people, and therefore I am wondering how I can get this?

I see that you have to be over 25 to be able to get IVF, what kind of options do we have when we are 19 and 20 years old, and pergotime has no effect? Do you have a background with mental health problems (anxiety), does this have anything to do with whether we can get help or not?

I'm a little worried that we'll have to put our dream on the shelf just because of my background and because we're young.

  Normally, anxiety or depression will not necessarily affect your ability to get help if needed.
  
  If you are not ovulating and are of normal weight, I would suggest that you ask your GP to refer you to a gynaecologist who will be able to assess your situation and follow you through a period of stimulation attempts. During the stimulation you should be followed up with ultrasound or blood tests so that it is known that you are responding to the stimulation with egg development and ovulation. In my opinion, this does not currently need to go as far as IVF, but we also offer such treatment to young couples if they otherwise meets the requirements of the law to receive such treatment.

  Hi Lita. It depends a bit on what you mean by support.
  
  It is probably rare for a public clinic to offer treatment to someone who is 40 years old. They usually have an age limit of 38, but I have heard that some also offer to those who are 39. This may vary from clinic to clinic, and perhaps also depending on whether they have a long waiting time or not.

I am 42 and my partner is 35, and we have been trying to get pregnant for 5 months. I understand that at my age you should start with IVF if you have not been successful after 1/2 a year. I want to try IVF in January (7th month), but my partner thinks we can wait 1-2 more months. I have regular menstruation (27-28 day cycle) and positive results on the ovulation tests I have taken the last three cycles. Ultrasound of the uterus/ovaries 1 year ago still showed good egg reserves and normal conditions. Neither of us have children before, but I have been pregnant once. Does 1-2 months from or to mean much at my age, or should we start immediately after 6 months? And how big is the chance of getting pregnant naturally?

Hi Marta. We believe that the upper age limit for the man should be around 55 years old, and in any case not over 60. The reason for this is that we believe that the child who is born as a result of the treatment should be able to have a father while it grows up.

Hi Anette! I have spoken to a doctor here regarding your question. We can see something about the number of eggs with an ultrasound, but a blood test to measure AMH can also help to say something about how many eggs are left. This does not necessarily give an answer to the chances of getting pregnant. It would also be a good idea to measure FSH early in the cycle to be able to get a better picture of the chances of success. See also a separate post about FSH and AMH earlier in the blog.

The fact that you got pregnant quickly at an age (42-43 years old) certainly doesn't count negatively, but that doesn't mean it's easy now.

We normally do not offer treatment when the woman is 44 years old, but if you are around this age, it may be a good idea to contact us to see if blood tests can be taken and a thorough and individual assessment can be made.

Can you see on ultrasound how many eggs are left and based on that how big the chance is of success with IVF?

    The chance of getting pregnant with your own eggs when you are 47 years old is very, very small. You are also over the upper age limit for assisted reproduction. Egg donation would be the best option for achieving pregnancy, but due to your age you would have to go abroad to get this.

When the man is sterilized, an examination must be performed to see if there are sperm cells in the testicular tissue, a relatively minor procedure. This can be done at Medicus. If sperm cells are found, in vitro fertilization can be performed. An examination of the woman must also be performed. After the examination, the doctor will consider whether we can offer treatment. Hi! This probably won't be possible in Norway. We have an upper age limit of 44 years, and as far as we know, no other Norwegian clinics practice a higher age limit than that. It is possible that there are foreign clinics that can offer it.
